This quick and easy activity is 100% confidential. No one at the County can see your answers. We encourage you to answer openly and honestly. Your participation guides the continued creation and improvement of wellness resources for all County employees. This assessment is crucial to better understand employee needs.
The Online Health Assessment is also your first step in completing the Wellness Dividend Program. If you are enrolled in a County Medical Plan, you are eligible to receive $500-$750! Don’t miss out on your money!
