Please note that the above information is needed for PDA records and for PDA to be able to contact you outside of county e-mail/pony mail. It is also needed to enroll you for membership in PORAC and long term Disability. Information provided will be shredded once your information is inputted into the PORAC system and will not be shared other than as indicated. Failure to provide the requested information in a timely manner may result in PDA’s inability to properly enroll you in PORAC and Long Term Disability.
